0536 - the Effect of Radio-opacifiers in Bone Cement on an in Vivo Model of Aseptic Loosening

چکیده
INTRODUCTION Aseptic loosening is the commonest cause of failure of total joint replacements. The pathogenesis of this process remains controversial but appears to involve prosthetic migration, joint fluid pressure and the host response to particulate wear debris. The Swedish Hip Registry has shown that the type of bone cement used effects the risk of revision for aseptic loosening. It has been suggested that this effect is mediated by the host response to the type of radio-opaque agent contained within bone cement particulate debris, as CMW, which contains barium sulphate (BaSO4) caused more bone resorption than Palacos, which contains zirconium dioxide (ZrO2), in an in vitro assay. Radiolucent bone cement particles caused less bone resorption than either. However, the proprietary bone cements differ in their chemical constituents by more than just the radio-opacifier they contain.

Proponents of the biological theory of aseptic loosening have in recent years tended to concentrate on the production and distribution of particulate ultra-high-molecular-weight polyethylene (UHMWPE) debris around the potential joint space. However, mechanical loading of cemented implants with the differing elastic moduli of metal stems, polymethylmethacrylate (PMMA) cement and bone can result ...
متن کاملHypersensitivity in aseptic loosening of total hip replacements. The role of constituents of bone cement.
Aseptic loosening is seen in a significant proportion of cemented total hip replacements (THR). In a small subgroup of patients who suffer early loosening polyethylene debris is unlikely to be responsible. We recently reported one case of allergic contact dermatitis to N,N-dimethylparatoluidine (DMT), an accelerator used in bone cement. We have therefore investigated this using skin-patch tests...
متن کاملEarly aseptic loosening of a mobile-bearing total knee replacement
Background and purpose - Registry-based studies have reported an increased risk of aseptic tibial loosening for the cemented Low Contact Stress (LCS) total knee replacement compared with other cemented designs; however, the reasons for this have not been established. We made a retrieval analysis with the aim of identifying the failure mechanism. Patients and methods - We collected implants, cem...
متن کاملTechnique to Improve Cement Penetration in Total Knee Arthroplasty
In total knee arthroplasty, infection has always been cited as being the number one cause of failure. However, more recent studies have shown aseptic loosening as the top reason for failure now. Proper cement technique is an important factor in reducing the occurrence of aseptic loosening. Clean and dried bone are important in achieving rigid cement fixation. Subchondral cysts may be hidden bel...
متن کاملAnalysis of Cement Mantle in Relation to Surgical Approach
Aseptic loosening remains the commonest cause of failure of total hip arthroplasty. Cement mantle defects are associated with aseptic loosening. Cementing technique has a profound influence on the incidence of aseptic loosening of total hip replacements. Two specific measures that seem to have the greatest impact on the longevity of cemented femoral stems are pressurization of cement and contro...
